Schedule:Monday-Friday 8am-5pm

Job Description:

• Ensures continuity of care to the patients and families of the designated coverage area, supplementing care provided by RN Case Managers.
• Assesses and evaluates patient’s status by evaluating patient and family/caregiver needs, writing and initiating the plan of care, and revising the existing plan of care as necessary.
• Timely performs focused assessments of the patient’s existing or acute needs as assigned by the Branch Administrator or Patient Care Coordinator. Assesses the immediate needs and concerns of the patient and family/caregiver and responds accordingly. Communicates interventions and changes to the care plan with the Case Manager and members of the interdisciplinary team.
• Completes the initial comprehensive assessment of patient and family to determine hospice needs. Performs a physical assessment and history of current and previous illness(es).
• Uses health assessment data to determine nursing diagnoses.
• Develops a care plan that establishes goals, based on nursing diagnoses, and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the plan of care.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient and family as appropriate per assessment and plan.
• Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
• Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.
• Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as
appropriate.

• Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s
condition to obtain physician’s orders, as required.
• Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
• Provides and maintains a safe environment for the patient.
• Assists the patient and family/caregiver and other team members in providing continuity of care.
• Works in cooperation with the family/caregiver and hospice interdisciplinary team to meet the emotional needs of the patient and family/caregiver.
• Communicates with the patient’s RN case manager and interdisciplinary team to ensure equipment, supplies, and services are available.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in service classes.
• Fulfills the obligation of requested and/or accepted assignments.
• Actively participates in quality assessment performance improvement teams and activities.
• Other duties as assigned.

Job Requirements:

• Registered Nurse licensed in the state of employment
• Certified Hospice and Palliative Nurse and/or BSN preferred
• Minimum 1 year of hospice experience required, 2+ years preferred
• Maintain an active Basic Life Support CPR certification
• Excellent verbal and written communication skills
• Self-directed and able to work with minimal direct supervision
• Valid driver’s license, car insurance, and reliable car
