Schedule:Monday, Wednesday, Friday's 2pm-6pm & Tuesday and Thursday's 245pm-645pm

Job Description:

Assist with personal care and hygiene needs, prepare meals and snacks, assist with her laundry, and cleaning her living areas, assist with activities of her choice in home or in the community.

Job Requirements:

CNA CERTIFICATION IS NOT REQUIRED!!! DSP is a Direct Support Professional formerly known as a DCW or PCA worker. Qualifications include High School Diploma or GED; CPR, TB Skin Test, Auto Insurance, one (1) year of experience caring for patients. Must not be listed on the OIG Exclusion list, Excluded Parties List System (EPLS).

Patient requires assistance with ADL's; needs reminders to get ready for the day and upkeeping of hygiene; duties include assisting with household chores, laundry, taking out into community to shop and eat; has to be picked up from day hab on Tuesday's and Thursday's at 245pm and will be at home on Monday, Wednesday, and Friday's
